Are you an experienced Care Coordinator or Field Care Supervisor looking to take the next step within complex care?

We are looking for a Complex Care Coordinator to join a growing healthcare provider in Wolverhampton, supporting the delivery of specialist community care packages across the West Midlands.

You'll play a key role in keeping complex care packages running smoothly, combining rota coordination and day-to-day operations with regular field visits, staff supervisions and client reviews.

This is a varied position where no two days are quite the same, making it ideal for someone who enjoys problem-solving, building relationships and working in a fast-paced care environment.

The role

As Complex Care Coordinator, you'll support community-based packages for adults and children with complex health needs, including tracheostomy care, ventilation, PEG feeding, spinal injuries and acquired brain injuries.

Your responsibilities will include:

• Creating and managing staff rotas to ensure care packages remain fully covered

• Coordinating with clients, families, carers and the wider operations team

• Managing last-minute staffing changes and resolving issues quickly

• Conducting staff supervisions, appraisals and team meetings

• Supporting the onboarding and compliance of new care staff

• Carrying out regular field visits across the West Midlands

• Attending MDT meetings and client reviews alongside clinical and operational colleagues

• Supporting safeguarding, incident reporting and continuous service improvement

• Liaising with healthcare professionals and commissioners

• Participating in a shared on-call rota, including one weekday evening and approximately one weekend in every 3–4

What we're looking for

To be considered for this Complex Care Coordinator position, you should have:

• Previous experience within care coordination, field care supervision or a similar healthcare role

• Experience within complex care or domiciliary care would be highly beneficial

• Good understanding of CQC requirements and person-centred care

• Strong rota management and organisational skills

• Confidence communicating with care staff, clients, families and healthcare professionals

• Ability to remain calm and find solutions when dealing with staffing issues or emergencies

• A full UK driving licence and access to your own vehicle due to regular field visits
