Are you an experienced Social Worker looking for your next long-term contract within a supportive 0–25 Disabilities Team?

We are working with a well-established local authority in North Wales that is seeking an experienced Social Worker to join its dedicated disabilities service. This is an excellent opportunity to support children, young people and young adults with complex needs, while benefiting from flexible working arrangements, stable leadership and manageable caseloads.

The team supports individuals aged 0–25, helping to ensure that children and young people with disabilities receive the right safeguarding support, assessments, care planning and services to promote their wellbeing, independence and development.

About the Service

You will join a dedicated Disabilities Team supporting children, young people and young adults aged 0–25 with a range of complex needs. The service is committed to child- and young person-centred practice, positive outcomes and close partnership working with families, education, health and other agencies.

The team places a strong emphasis on ensuring that every child or young person has access to the tailored support, resources and care they need to thrive.

Key Responsibilities

  • Manage a caseload of children, young people and young adults aged 0–25 with disabilities and complex needs
  • Complete Child and Family Assessments, statutory assessments and reviews Undertake Care Act, transition and Short Break assessments where appropriate
  • Develop, implement and review person-centred care and support plans
  • Work closely with families, schools, health professionals and multi-agency partners Identify, assess and manage safeguarding concerns in line with statutory guidance
  • Support Child in Need, Child Protection and Looked After Children processes where required
  • Contribute to transition planning for young people moving into adulthood Prepare clear, high-quality assessments, reports and court documentation when necessary
  • Ensure the views, wishes and feelings of children and young people remain central to all planning and decision-making

About You

  • Qualified Social Worker Registered with Social Work England
  • Recent experience in a Children with Disabilities, 0–25 Disabilities, Safeguarding, Child in Need, Assessment or Transition Team
  • Strong knowledge of safeguarding legislation and practice relating to children and young people with disabilities
  • Experience supporting children and young people with complex needs
  • Confident completing assessments, care plans and safeguarding work
  • Skilled in relationship-based practice and multi-agency partnership working
